I'm preparing to file 3 other bugs to ffmpeg in related matters, I already reported 4 of which 2 are still not fixed.
Haven't time to further test your, but:
I think if you cut the INPUT video & audio both in lenght & quality you'll see the same, issue, and it will be easier for bug reporting.
For you to test
1/ You encode to aac, I have many issues with aac, give a try with mp3
2/ Test as well mkv as container instead of mp4.
3/ encode video in mpeg4 instead of libx264, which also gives issues
however my main purpose is to create 3 different quality video (MP4 file) with the source video and audio
1. Best quality
2. Medium Quality
3. Normal/Average/low quality
format must be mp4 and aac

so, it would be great if you can provide me better CLI that works on all player?
